Now that you have been scheduled for a Diagnostic Evaluation, we are providing you with the following documents:
- Notice of Privacy Practices / Aviso de prácticas de privacidad;
- Client Rights & Grievance Procedure / Derechos del cliente/paciente Política de quejas
- Expectations for Participation in OhioGuidestone Programs & Services / Expectativas de participación en los programas y servicios de OhioGuidestone; and
- Your Rights and Protections Against Surprise Medical Bills / Sus derechos y protecciones frente a facturas médicas sorpresa.
Please review these documents prior to your Diagnostic Evaluation appointment.
If you have any questions about any of the paperwork you receive or have difficulty completing any of the forms, you may ask your provider at your appointment.
After the completion of the Diagnostic Evaluation, your care will be assigned to a provider. If you have questions following the completion of your Diagnostic Evaluation, please reach out to us at 844.6.CALL.OG (844.622.5564).
